Adding Pages to your US Passport Emergency services

• make a personal check or Money Order for $142 payable to U.S. Department of State.


Our Fees

• Our Service Fee is $175.

• For return shipping Fee, choose one of the following.

       * A prepaid, filled in airbill from Federal Express.
       * $25 for priority overnight.
       

• If you're paying by check or money order, please make the check payable to "Deluxe Passport Express".

• If you're paying by credit card, please make sure you entered your credit card information on the customer information form.(See Below)

6)Services order Form

• Complete and submit the services order form. Once you submitted the form, print a copy and send it along with rest of the documents to Deluxe Passport Express. If you're paying by credit card make sure to sign the printed customer form.

7)Shipping

Visit one of our walk-in offices, or ship the following via FedEx/UPS:

1. Signed Application Form DS -4085
2. Valid Passport
3. 2 Letters Of Authorization
4. Travel Document
5. Government fee payment
6. Services Order Form


All signed documents must have original signature.

Call us at 718-387-5441 to indicate that you're processing an Emergency Passport. Overnight it to Deluxe Passport Express, 57 Lee Ave, Brooklyn NY 11211. Our office hours are Monday through Thursday 9:00am to 6:00 pm and Friday from 9:00am to 1:00 pm EST.

We strongly recommend that you send your documents via FedEx overnight.
