Standard US Passport Renewal Instructions
1) Complete and Print Application Form DS-82
• Click here to complete the application form online. Completed application must have applicant's original signature.
Please note: handwritten applications are not accepted. Application have to be completed online, and printed single sided.
2) Passport
• Your passport should be issued within the past 15 years.
• You must've been over the age of 16 at the time your passport was issued.
• If your name has changed since your passport was issued, present a certified copy of legal document specifying your name change (e.g. marriage certificate, divorce, adoption decree, or court order)
3) Passport Photo
You must provide one 2 x 2 inches passport photo. See U.S. Passport
Photo Regulations.
4) Letter
of Authorization
Print, complete and sign a Passport Authorization Letter.
5) Fees
• Make a personal check or Money Order for $190 payable to U.S. Department of State. (Starter's checks not accepted)
• Our Service Fee is $95
• Shipping fee is $20.
• Credit card information should be entered on the service order form (see below).
• When paying by money order, make it payable to "Deluxe Passport Express".
6) Services Order Form
• Complete the Service Order Form. Print a copy and send it along with rest of your documents.
