Please collect and send the following documents:

1) Valid US Passport

You must send your valid, signed US Passport, valid for at least 6 months, and has at least 2 blank visa pages. If your passport does not meet these requirements, you must renew your passport or obtain a new one.

2) One Passport Photo

Photos must be 2x2, with a plain light or white background. Only front view of face acceptable.

3) Visa Application Form

4) Additional Particular Form

Click here to access the additional particular form. Complete form, print and sign.

5) Proof of Address

You must provide a photocopy with proof of address that matches the present address on the visa application. Acceptable proof is either a state issued ID, or major utility bill.

6) Fees

Consular Fee

Prepare a Money Order payable to "Cox & Kings Global Services USA LLC" for one of the following fees:

Our Fee

Our service fee is $125.00.

• For return delivery, add $30.00 for overnight shipping.

• If you are paying by money order, please make it payable to "Deluxe Passport Express".

• If you're paying by credit card, please make sure to enter your credit card information on the customer information form. (See Below)

7) Customer Information Form

• Complete the customer information form. Print a copy and send it along with rest of your documents.

8) Shipping

Call us at 718-387-5441 to indicate that you're processing a Chinese Visa. Overnight all of the above documents to Deluxe Passport Express, 57 Lee Ave, Brooklyn NY 11211. Our office hours are Monday through Thursday 9:00am to 6:00 pm and Friday from 9:00am to 1:00 pm EST.

We strongly recommend that you send your documents via FedEx overnight.
