Please collect the following:
1) Valid US Passport
You must send your valid, signed US Passport, valid for at least 6 months, and one blank visa page. If your passport does not meet these requirements, you must renew your passport or obtain a new one.
2) Passport Photo
Photo must be 2x2, color or black and white with a light or white
background. Only front view of face acceptable.
3) Visa Application Form
Print, complete, and sign the Visa Application Form.
4) Fees
Consular Fee
Make a Money Order payable to the "The Consulate of Vietnam in New York", Do not send personal or company checks.This fee may also be paid together with our service fee to "Deluxe Passport Express"; or by credit card, by entering your credit card information on the customer information form (see below).

Our fee
Our service fee is $75.00.
• For return delivery, choose one of the following:
* $30 for overnight shipping
* A prepaid filled in airbill from FedEx or UPS
* Use Your FedEx Account Number
* Pickup in person
• When paying by money order, make it payable to "Deluxe Passport Express".
• Credit card information should be entered on the Service Order Form (See Below).
5) Customer Information Form
• Complete the customer information form. Print a copy and send it along with rest of your documents.
6) Shipping
Call 718-387-5441 to inform us that you're sending a Vietnam Business visa. Visit one of our walk-in offices, or ship the above via FedEx/UPS to: Deluxe Passport Express, 57 Lee Ave, Brooklyn, NY 11211. Our office hours are Monday-Thursday 9:00am - 6:00pm, Friday 9:00am - 1:00pm EST.
We recommend that you send your documents via FedEx/UPS.
